LAPAROSCOPIC GYNECOLOGICAL SURGERY DURING THE COVID PANDEMIC

In order to prevent Coronavirus transmission in Healthcare facilities, there has been major changes in the way Surgical procedures are performed today throughout the world. We have responded to the upheaval in a positive way and have adapted our surgical practices to the need of the hour.

Global Scenario of Laparoscopic Surgery during the COVID pandemic

  • Most Apex surgical organisations (AAGL, RCOG, BSGE, IAGE) have pronounced that Laparoscopic surgery is a safe surgical modality during the pandemic.
  • Certain precautions have been universally agreed upon and these guidelines have been the cornerstone on which we have based our operating room modifications.

  • We routinely perform preoperative COVID testing on all patients so that nobody is subjected to any undue risk
  • All our theatre staff wear full PPE during surgery so that neither the patient nor the staff are exposed to the infection.
  • All our Laparoscopic operations take place at Anandaloke Hospital, Sevoke road, Siliguri. This is to ensure uniform standards in the operating room at all times.

Additional measures we have adopted to make it even safer for our patients

  • We do our surgeries at minimal intra-abdominal pressure (10-12mm).
  • We have limited our use of ‘energy sources’ and have used “LAPAROSCOPIC SUTURING more frequently than before, resulting in minimal aerosol generation.
  • We routinely employ impervious ‘ENDOBAGS’ for retrieval of surgical specimens from the abdominal cavity. ‘In-bag’ Morcellation of the surgical specimen is standard practice in our centre.
  • The pneumoperitoneum is completely deflated by the smoke evacuation system before the Endobag is introduced into the abdomen as well as during its retrieval.

Cyst placed in an Endobag prior to retrieval

New practices introduced in our Operation Theatre

Closed Smoke Evacuation system :

  • Potentially harmful Surgical smoke generated in the operative field is sucked out, passed through a virucidal solution onto the central suction system.
  • Inadvertent gas leakage is practically nil in our theatre
